Please click on links below to access consent forms which require to be completed prior to pupils having medication in school. These forms can be downloaded to complete and are also available from the school office.
Any medication held in school will be returned at the end of the school year and we would request that parents/carers contact the school office in August each year if medication is required to be held in school for the current school year.
It is the responsibility of parents/carers to advise the school throughout the year if there are any changes to pupils medication and to ensure that medicine supplied to school is within the expiry date and replaced when required.
