Medical Forms

Many pupils will, at some time in their school careers, need to take medication in school.  For the vast majority this will be a short-term requirement, with pupils simply finishing a course of medication which has been prescribed for them by their family doctor.  Other pupils have medical conditions such as asthma or diabetes which may require long-term support and, if their conditions are not properly managed by taking regular medication, their access to education could be limited.

Parents are required to complete a form which then allows school to administer the medication to the child.  All medication (other than inhalers) must be handed in to and collected from the school office. No medication (other than inhalers) should come to school in a child’s school bag. Medication cannot be administered to a child unless the appropriate form is filled in and signed by a parent/guardian.

School cannot store medication for long periods of time, if your child is taking medication for a short period of time, parents are asked to supply weekly or monthly supplies of the medication.  Medication which requires routine administration e.g creams, inhalers can be stored in school for longer periods, however it remains the responsibility of the parents to ensure that the medication is replaced before the expiry date.  All medication must be in the original container and must have the name of the child, the name of the drug and its expiry date, the dosage – level and frequency.

Prior to the end of the summer term all medication should be collected by an adult as we cannot store medication in school over this period.
