Yesterday your child will have brought home a form from the NHS regarding the above immunisation.
Health staff will call at the school early next week to collect the completed forms. Please can these be returned by NEXT MONDAY 2nd SEPTEMBER at the latest.

Please note that the day we have for the immunisations taking place is TUESDAY 22nd OCTOBER.
