Applications forms can be downloaded from the website, SMHW and from the school computers on ‘Public-> Captain information-> application form’.

Paper copies are available from the main office and guidance offices.

Remember to tick whether you are applying for school captain, house captain or both.

School captains will be appointed first, followed by house captains.

Completed application forms must be returned by Friday 27th April. No late applications accepted!
