How to request support
The joint ASN Outreach Services accept support requests from educational establishments through the Staged Intervention process. Health services supporting children and young people with ASN also request support from the service.
Support Request Meeting Form Completion Guidance
- For individual children/young people.
- For Gypsy/Traveller children/young people.
- For Home/Hospital support.
- For all other support.
All support requests should be submitted electronically to a new shared mailbox: asnrequest@stirling.gov.uk
Support requests should be made using Child’s Plan Forms 1, and 2 or 3. We no longer require the relevant parts of form 7 as standard. If you have any other relevant paperwork from the GIRFEC Child’s Plan suite this can be included.
Please ensure Support Requests are sent to the mailbox in a zipped file. The file name must state the name of the child, and the team from whom support is being requested, e.g. asnsupportrequest.jbloggs.sebn
1 Requests for support for individual children/young people.
For all requests for support regarding individual children and young people from the ASN Support Services, please send the child’s plan forms 1, and 2/3. If other forms are already completed, for example, forms 4 and/or 6, these can be submitted if they have relevant information to support the request. Parents/carers are partners in this process and should be aware of this request, as should the child. The forms should have the required detail with the appropriate, relevant information to support the request.
Please indicate clearly the ASN Support Services team you are seeking support from. You may request support from more than one team. The rest of Form 2 should provide relevant information to support the request.
There are two drop-down boxes for the Purpose and Agency. You should select, Request Assistance and Education Services. In the text boxes you should then put the name of the team that you are requesting assistance from and ASN Support Service or SEBN Support Service. Please see attached appendices 4 and 5 for worked examples. The teams are as follows:
– Early Years Support for Learning (From age 3- Primary School)

Please complete the Notification of Gypsy/Traveller Children/Young People form with the educational establishment details including the point of contact and the children/young people’s basic details.
TheChild/Young Person’s details section can be duplicated if there is more than one child. (gypsy traveller notification)
- For Home/Hospital support.
Please complete the Notification of long term absence due to medical reasons form with the educational establishment details including the point of contact, the children/young people’s basic details, information regarding the hospital and/or illness and duration of absence. (long term absence)
- Requests for all other support – not for individual children/young people.
Requests for all other support that is not for individual children/young people can be made using the ASN Support Services, Support Request Form. (support request form). No other paperwork is required.
Using this form the support requested will be:
- Consultation
- Staff development
- Group work
Make it clear what is the support that is being requested and the purpose and the impact of this support.
Using this form, you can request support for input from the following teams:

For any direct input from ASN Support Services, the child/young person requires to be on a minimum of stage 2 of staged intervention. For some initial observation/assessment for Sensory Impairment and EAL, requests can be accepted at stage 1 with the expectation that if further direct input is required, the child /young person will be moved to stage 2. Please refer to the ASN Support Service Guidance for specific information regarding each team.
The Support Request Group meets regularly across the session. Meeting dates are sent annually to all educational establishments. Requests must be submitted by the given date. Following the meeting a letter will be emailed to the referrer detailing the outcome. If you have not received a letter within 10 days of the meeting please contact the relevant person as follows:
