Medication in School

If your child needs to have medication given to them in school, one of the following forms must be printed out,  filled in and handed into the school office.

  • All medications brought into the school must be in the original packaging and include the chemist label attached.
  • Medication will not be administered without a completed form.

Medication given to a child supported by administration (first-aider), please use this form:

Medication that the child takes by themselves, please use this form:

GSHN Appendix 5 Parental Request Form – Self Administering
