The identity of the lead professional in a deaf young person’s transition from school to further education, university, training or work, will vary according to local practice. A Template for Success may be particularly useful for that lead professional, but it will also be of use to any professional playing a direct role in supporting a young person’s transition. In particular, the template is designed to support professionals to think about, and understand, how they fit into the wider transitions process.”
