What is transition?

Every child goes through transition stages as they move through school education.  The first transition takes place when a child enters pre-school.  This is followed by the transfer to primary school and then from primary to secondary provision.  The final transition takes place when a young person leaves school to enter the next stage of education the world of work.

Transition for Children and Young People who require Additional Support.

If your child has additional support needs it is very important that each transition is as smooth as possible and that communications between everyone involved are as effective as possible.  At each stage of the transition process you will be fully consulted about your child’s needs and involved in the planning for them.  Information will be gathered from all the professionals who are involved to enable detailed planning to take place.
