It has been a busy couple of weeks in the school getting into the new academic session, and to ensure that we have all our records and files all current and up to date, to allow access to systems and to capture your opinions, we have sent out the following forms for your completion and return;
- Annual Data Check (paper form),
- Parental Consent Form Detailing Health and Medical Information (Walks Around Ochiltree),
- Parental Consent Form – Trip to Darnlaw Farm, RHET (P3/4 only),
- Flu Immunisation Consents (forms should be completed and returned in sealed brown envelope even if consent is not given)
- Photographic Consent
- EAC Acceptable Use of Computer Facilities
- Learning Journals – Form for completion
- Primary 7 Residential – Form (P7 Parents only)
We note that there are few outstanding awaiting return, and would appreciate that these forms be completed online or handed back to school as soon as possible.
