mceNonEditable fas\">\uf0ad<\/span>\u00a0<\/strong><\/span><\/p>\n<h3>What are they?<\/h3>\n<p>&#8216;Personal Plan&#8217; is an umbrella term for a suite of documents providing information about a child, informing how the setting meets all of the child&#8217;s needs.\u00a0 This personal planning should enhance the quality of daily life for the child.<\/p>\n<p><cite>&#8220;My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met as well as my wishes and choices.&#8221;<\/cite><\/p>\n<p>(<a PRINCIPLES:<\/strong><\/span><\/p>\n<p><strong>Being Included:<\/strong>\u00a0 This is about putting the child at the centre of the plan.\u00a0 The <a href=\"https:\/\/blogs.glowscotland.org.uk\/sb\/earlylevelportal\/childs-voice\/\" rel=\"noopener\">child should be listened to<\/a> and ensure that their needs, preferences and wishes have been considered.\u00a0 Often <a href=\"https:\/\/blogs.glowscotland.org.uk\/sb\/earlylevelportal\/family-partnerships\/\" rel=\"noopener\">parents and carers<\/a> will be the child&#8217;s spokesperson, but every opportunity should be taken to find out information from the child directly.\u00a0<\/p>\n<p><strong>Promoting Positive Outcomes:\u00a0 <\/strong><a href=\"https:\/\/blogs.glowscotland.org.uk\/sb\/earlylevelportal\/nurture\/\" rel=\"noopener\">Nurturing<\/a> relationships that encourage quality conversations are vital to ensure that children can shape their support for achieving personal targets or goals.\u00a0 The achievement of these goals promotes increased <a href=\"https:\/\/blogs.glowscotland.org.uk\/sb\/earlylevelportal\/health-and-wellbeing\/\" rel=\"noopener\">health and wellbeing<\/a>, and self-esteem.\u00a0 The progress towards, and achievement of, Childcare settings (ELC).<\/li>\n<li>This information must be up to date and reflects the current needs, interests and stages of development.\u00a0 The plan is collated using information from the child and their family or carer.<\/li>\n<li>Personal Plans are reviewed (with parents or carers) and signed at least every six months or whenever a change has been identified (<a href=\"https:\/\/blogs.glowscotland.org.uk\/sb\/earlylevelportal\/medication\/\" rel=\"noopener\"><strong>Medication records<\/strong><\/a> every three months).<\/li>\n<li>Parents should be asked to complete a new care plan annually to ensure information has been thoroughly reviewed and is meeting the current needs of the child.<\/li>\n<li>Practitioners have a shared understanding of the information about a child and use it to ensure that each child is supported to progress and be the best they can be.<\/li>\n<li><a href=\"https:\/\/blogs.glowscotland.org.uk\/sb\/earlylevelportal\/care-plans\/\" rel=\"noopener\"><strong>Care Plans<\/strong><\/a>, registration documents,\u00a0<a href=\"https:\/\/blogs.glowscotland.org.uk\/sb\/earlylevelportal\/chronologies\/\" rel=\"noopener\"><strong>chronologies<\/strong><\/a> and medication forms should be kept securely.<\/li>\n<li>All information held on individual children must be shared with parents and carers.<\/li>\n<li>This information may be used to ensure practitioners are deployed to meet the needs of individual children.\u00a0\u00a0<\/li>\n<li>If children attend more than one ELC provider, information is shared regularly to effectively support the child&#8217;s health, wellbeing and progress.<\/li>\n<\/ul>\n<ul>\n<li><strong>During periods of transition (horizontal or vertical), there should be a shared professional responsibility for continuity of learning, care, support and challenge, and sharing essential information appropriately and in good time.<\/strong><\/li>\n<\/ul>\n<ul>\n<li>Personal Plans should reflect the principles of <a href=\"https:\/\/blogs.glowscotland.org.uk\/sb\/earlylevelportal\/getting-it-right-for-every-child-girfec\/\">GIRFEC<\/a> and the <a href=\"https:\/\/www.unicef.org.uk\/what-we-do\/un-convention-child-rights\/\" target=\"_blank\" rel=\"noopener\">UNCRC<\/a> (Care Plan), safeguarding and promoting <a href=\"https:\/\/blogs.glowscotland.org.uk\/sb\/earlylevelportal\/childrens-rights\/\">children&#8217;s rights<\/a> in line with national and international policy and guidelines.<\/li>\n<li>Personal Plans should record achievement and success, for example: learning journeys.<\/li>\n<li>Personal plans are a tool for safeguarding, <a href=\"https:\/\/blogs.glowscotland.org.uk\/sb\/earlylevelportal\/child-protection-2\/\" rel=\"noopener\">child protection<\/a> and managing risk.<u><\/u><\/li>\n<li>Personal plans are regularly audited to ensure they meet the Health and Social Care Standards and the child&#8217;s needs.<\/li>\n<li>Personal Plans should reflect the 5 principles of the Health and Social Care href=\"https:\/\/hub.careinspectorate.com\/media\/4212\/qa-calendar.pdf\" target=\"_blank\" rel=\"noopener\">Care Inspectorate<\/a>.<\/p>\n<p>Practitioners use the <a href=\"https:\/\/glowscotland-my.sharepoint.com\/:b:\/g\/personal\/earlylevelportal_glowmail_org_uk\/Ed7zH1HrTvVLiBM3BqHLHmYBPNzlzrpdp8CmH33bU-Azxg?e=MHheAW\" target=\"_blank\" rel=\"noopener\"><strong>Quality Improvement Framework for Early Learning and Childcare Sectors<\/strong><\/a> (Education Scotland and Care Inspectorate September 2025) to self evaluate against the &#8220;Nurturing Care and Support&#8221; quality indicator, which includes a personal planning theme (p47).<\/p>\n<p>All practitioners are familiar with the Care Inspectorate Guide For Providers on Personal Planning.\u00a0 Setting managers ensure that personal plans meet the current Health and Social Care Standards and expectations.<\/p>\n<p>The setting has an agreed personal plan format for each child, including Care Plans, chronologies, learning journeys, medication and other proforma.
