We regularly assess each child&#8217;s individual needs and evaluate the effectiveness of our transition strategies. We ensure consistent relationships to support smooth transitions and minimise disruptions to children&#8217;s care and learning experiences.<\/p>\n<p>\u00a0<\/p>\n<p><strong>Nurturing Care &amp; Support \u2013 p47 Personal Planning<\/strong><\/p>\n<p>A child-centred approach guides our strategies for transitions. We place children and families at the heart of any decision-making process.
