This includes an element<br \/>\nof personalised support but is provided within the<br \/>\nservice. The agency will co-ordinate and monitor the<br \/>\nsupport to overcome the barrier to flourishing.<\/p>\n<p style=\"text-align: center\">\n<strong>Stage 2<\/strong><br \/>\nSometimes children and young people need extra<br \/>\nsupport within universal services. This additional<br \/>\nsupport should be determined by a wellbeing<br \/>\nassessment and informed by the views and wishes of<br \/>\nthe child or young person and their parent or carer.<br \/>\nThe universal service practitioner will initiate a single<br \/>\nagency Child\u2019s Plan and chronology.<\/p>\n<p style=\"text-align: center\">\n<strong>Stage 3<\/strong><br \/>\nStage 3 supports are not needed by most children<br \/>\nand young people. Targeted supports at this level go<br \/>\nbeyond what is available via universal provision and<br \/>\ninvolve at least one other agency. With the agreement<br \/>\nof the child or young person and their parent or<br \/>\ncarer, a wellbeing assessment will be undertaken<br \/>\nto identify needs and specify additional supports<br \/>\nrequired to overcome these. A multi-agency Child\u2019s<br \/>\nPlan is necessary at stage 3 to co-ordinate supports<br \/>\nand clarify professional roles and responsibilities. A<br \/>\nmulti-agency Child\u2019s Plan meeting will be convened<br \/>\ninvolving the parents or carers, the child or young<br \/>\nperson (if appropriate), the universal service and<br \/>\npartner agencies and will agree who should be the<br \/>\n\u2018Lead Professional\u2019 for the child (see below).<br \/>\nIn some circumstances, the multi-agency meeting will<br \/>\nidentify a need for statutory measures to support the<br \/>\nchild or young person.<\/p>\n<p style=\"text-align: center\">\n<strong>Stage 4<\/strong><br \/>\nFor some children and young people, highly specialist<br \/>\nsupport is required from two or more agencies to<br \/>\naddress their wellbeing needs. At stage 4, the Lead<br \/>\nProfessional will almost always be a social worker.<br \/>\nIf the child or young person is involved in a child<br \/>\nprotection process and\/or is Care Experienced (looked<br \/>\nafter at home or away from home) or subject to a<br \/>\nCompulsory Supervision Order, the Lead Professional<br \/>\nwill always be from Social Work.<br \/>\nSupport at stage 4 is intensive and applied because<br \/>\nthe child or young person is experiencing significant<br \/>\nrisk. For these reasons, it is expected that support at<br \/>\nthis stage will be time limited. The Child\u2019s Plan must<br \/>\nbe SMART (specific, measurable, achievable, realistic<br \/>\nand time-limited) and should be reviewed regularly<br \/>\nand amended as necessary to maximise impact upon<br \/>\npositive outcomes for the child or young person.<br \/>\nAt stage 4, the ultimate aim of the Child\u2019s Plan is to<br \/>\nintervene minimally in the child or young person\u2019s<br \/>\nlife to reduce or remove barriers to the child or young<br \/>\nperson\u2019s happy and healthy development and to<br \/>\nstrengthen family capacity.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Renfrewshire&#8217;s GIRFEC Policy Stage 1 At the level of universal services, education and health work to support children and young people and their families, intervening early when necessary to prevent unmet needs escalating.
