{"id":371,"date":"2025-07-11T06:56:20","date_gmt":"2025-07-11T06:56:20","guid":{"rendered":"https:\/\/blogs.glowscotland.org.uk\/nl\/jigsawflc\/?page_id=371"},"modified":"2025-07-11T06:56:20","modified_gmt":"2025-07-11T06:56:20","slug":"duty-of-candour","status":"publish","type":"page","link":"https:\/\/blogs.glowscotland.org.uk\/nl\/jigsawflc\/duty-of-candour\/","title":{"rendered":"Duty of Candour"},"content":{"rendered":"<p><u>Jigsaw Family Learning Centre Duty of Candour Annual Report 2025 &#8211; 2026<\/u><\/p>\n<p>The Duty of Candour underpins the Scottish Government\u2019s commitment to openness and learning which is vital to the provision of the safe, effective and person-centred health and social care.<\/p>\n<p><strong>A Duty of Candour procedure should be followed when an unexpected or unintended incident has occurred, where the incident results in:<\/strong><\/p>\n<ul>\n<li>Someone has died<\/li>\n<li>Someone has permanently less bodily, sensory, motor, physiologic or intellectual functions<\/li>\n<li>Someone\u2019s treatment has increased because of harm<\/li>\n<li>The structure of someone\u2019s body changes because of harm<\/li>\n<li>Someone\u2019s life expectancy becomes shorter because of harm<\/li>\n<li>Someone\u2019s sensory, motor or intellectual functions is impaired for 28 days or more<\/li>\n<li>Someone experienced pain or psychological harm for 28 days or more<\/li>\n<li>A person needed health treatment in order to prevent them dying<\/li>\n<li>A person needing health treatment in order to prevent other injuries<\/li>\n<li><\/li>\n<\/ul>\n<p><strong>About Jigsaw Family Learning Centre: <\/strong><\/p>\n<ul>\n<li>Local authority centre with the registered capacity for 80 children at any one time.<\/li>\n<li>Extended year care of children is provided between the hours of 8am-6pm.<\/li>\n<li>24 ELC staff, including the senior leadership team.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>Procedures to follow in the case of an Incident where duty of candour would be actioned: <\/strong><\/p>\n<ul>\n<li>All incidents reported to HOC and local authority<\/li>\n<li>Reported as necessary to Care Inspectorate and NLC Health &amp; Safety<\/li>\n<li>Review of incident with involved staff<\/li>\n<li>Risk assessment carried out<\/li>\n<li>Adaptations carried out when necessary<\/li>\n<li>Wellbeing Support available for staff affected by incident<\/li>\n<li>Supports offered to children and families affected by incident<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<ol>\n<li>How many incidents have occurred to which the Duty of Candour applies?<\/li>\n<\/ol>\n<p>There have been 0 incidents between July 2024 \u2013 July 2025 to which Duty of Candour has applied.<\/p>\n<p>&nbsp;<\/p>\n<ol start=\"2\">\n<li>If so, have the correct persons been notified?
