Following a test of change, this has resulted in the launch of the new <a href=\"https:\/\/blogs.glowscotland.org.uk\/glowblogs\/public\/fvpp\/uploads\/sites\/9924\/2025\/12\/17095542\/FV-Notification-of-Concern-Form-Final.docx\" target=\"_blank\" rel=\"noopener\">Notification of Concern Form<\/a>. This new form will replace the Forms 2a and 2b as of Monday 1st December 2025.<\/p>\n<p style=\"text-align: justify\">The intention of this change is to make it simpler to share concerns regarding a child or young person and to ensure that sufficient information is provided to determine next steps. All practitioners are asked to phone social work first to discuss a concern.<\/p>\n<p style=\"text-align: justify\">We have also developed <a href=\"https:\/\/blogs.glowscotland.org.uk\/glowblogs\/public\/fvpp\/uploads\/sites\/9924\/2025\/12\/02102024\/FV-NOC-Guidance-Final-Nov-25.pdf\" target=\"_blank\" rel=\"noopener\">new practice guidance<\/a> to assist in the roll out of the Notification of Concern form.\u00a0 As on 1.12.25, the new form will be available here: <a href=\"https:\/\/blogs.glowscotland.org.uk\/glowblogs\/fvpp\/child-protection\/reporting-a-child-protection-concern\/\" target=\"_blank\" rel=\"noopener\">Reporting a Concern<\/a>.<\/p>\n<p style=\"text-align: justify\">Where a named person is seeking support from social work due to an accumulation of concerns or increase in risks, or because of a decision made at a Team Around the Child meeting, they may be asked to complete a <a href=\"https:\/\/blogs.glowscotland.org.uk\/glowblogs\/public\/fvpp\/uploads\/sites\/9924\/2025\/12\/02092357\/FV-Wellbeing-Assessment-25.11.25.docx\" target=\"_blank\" rel=\"noopener\">Wellbeing Assessment Form<\/a> to ensure that the information shared is as full as possible.<\/p>\n<p style=\"text-align: justify\">New <a href=\"https:\/\/blogs.glowscotland.org.uk\/glowblogs\/public\/fvpp\/uploads\/sites\/9924\/2025\/12\/02092735\/Forth-Valley-TAC-Guidance-Final-V5.pdf\" target=\"_blank\" rel=\"noopener\">Team around the Child Guidance<\/a> has also been developed, which we hope that you find helpful and would encourage you to refer to <a href=\"https:\/\/blogs.glowscotland.org.uk\/glowblogs\/fvpp\/girfec-in-falkirk-info\/girfec-res-paperwork\/#:~:text=Forth%20Valley%20TAC%20Guidance\" target=\"_blank\" rel=\"noopener\">Team Around the Child (TAC) Guidance &amp; Information Leaflets<\/a>.<\/p>\n<p style=\"text-align: justify\"><strong>Please ensure that the new notification of concern form and guidance are circulated widely across your agency for use from Monday 1st December 2025.<\/strong><\/p>\n","protected":false},"excerpt":{"rendered":"<p>As a result of Quality Assurance and Improvement work, the Forth Valley GIRFEC Working Group have reviewed and updated the form used to share a concern about a child or young person.
