It is vital that children and young people are able to put their views, if not within the meeting itself then through one of the Team Around the Child seeking and contributing their views.<\/p>\n<p><a href=\"https:\/\/blogs.glowscotland.org.uk\/fa\/public\/dennyhighips\/uploads\/sites\/3302\/2023\/12\/21120911\/Meeting-Record-Form-6.dot\">Meeting-Record-Form-6<\/a><\/p>\n<p><strong>Form 4:<\/strong> <strong>Child Action Plan<\/strong> is the document for recording intended and desired outcomes for the child\/young person. It also records the agreed actions to achieve those outcomes, including what support the child\/young person will receive from the team around them. The plan is crucial to the assessment process, within a context of plan-do-review. It describes the objectives, and how it is known they have been achieved, for the support provided, with a clear description of the interventions and strategies to support the child\/young person.
