Form 2A: Child’s Plan: Wellbeing Assessment is used to:
 Record the child or young person’s wellbeing;
 Record initial assessment and emerging concerns;
 Request assistance from another agency/service;

Resilience Matrix:  During SLG we will discuss and identify resilience and vulnerability, as well as adversity and protective factors by using a blank matrix and ‘plot’ on a blank matrix.  This assessment can then be used by Key Adult to complete the remaining pages of the Child’s Plan: Wellbeing Assessment (Form 2).

Form 6: Meeting Record is a structure for recording all meetings about children/young people, who was there and what action was agreed. It is vital that children and young people are able to put their views, if not within the meeting itself then through one of the Team Around the Child seeking and contributing their views.

Form 4: Child Action Plan is the document for recording intended and desired outcomes for the child/young person. It also records the agreed actions to achieve those outcomes, including what support the child/young person will receive from the team around them. The plan is crucial to the assessment process, within a context of plan-do-review. It describes the objectives, and how it is known they have been achieved, for the support provided, with a clear description of the interventions and strategies to support the child/young person. The plan will consist of the agreed actions and specify which ones will be undertaken by the Team Around the Child, the parents/carers and the child/young person.
