All children and young people who have needs that require individualised planning and targeted support delivered by school staff and\/or the JST (Stage 1 and 2) should have a child\u2019s plan (education).<\/p>\n
Children and young people whose needs can be met at the universal stage do not require to have an individualised plan as it would be expected that within a class there will be learners with a range of skills and abilities whose needs can be met through universal pupil planning. Good practice such as personalisation, groupings and differentiated materials would be considered typical practice of teachers and therefore would not constitute individualised and targeted support.<\/p>\n
The need for a lead professional and a child\u2019s multi agency assessment and plan (Stage 3) should always be considered when reviewing the child\u2019s plan (Education). A lead professional is required where two or more agencies work together to meet the needs of the child\/young person and family to ensure effective delivery of services. The role of the lead professional is to coordinate a multi-agency assessment, produce a child\u2019s multi-agency plan (CMAP) and lead the implementation of the plan.<\/p>\n
