Good practice such as personalisation, groupings and differentiated materials would be considered typical practice of teachers and therefore would not constitute individualised and targeted support.<\/p>\n<p>The need for a lead professional and a child\u2019s multi agency assessment and plan (Stage 3) should always be considered when reviewing the child\u2019s plan (Education). A lead professional is required where two or more agencies work together to meet the needs of the child\/young person and family to ensure effective delivery of services.
