Who the service is aimed at:
The Quarriers Family Support Service is aimed at Children and Young People with disabilities as well as young adults with disabilities.
There is also the Quarriers Parent Carers Service in place to support the Parents/Carers of children and young people under the age of 18.

The age range of the Quarriers Family Support Service is birth to 18yrs and we also work with young adults.
Area covered:
All areas of Dumfries and Galloway.
Can an individual access the service directly, or do they need to be referred to it by another service?
The Quarriers Family Support Service is referred to by a member of the Social Work team.
However, the Quarriers Parent Carers Service can be referred by a professional as well as self referrals from the Parent/Carers themselves.
How to make a referral (if applicable): (If a service is referring a child or young person, do they need to submit a Request for Assistance Form or does the service have its own forms that a referrer would need to complete?)
The Quarriers Family Support Service has it’s on referral form and this can be shared upon request.
The Quarriers Parent Carers Service has it’s on referral form and this can be shared upon request.
Top tips for making a ‘good’ referral (if applicable): (If a referral is needed to access this service, is there any particular information that you need on this?)
The referral is to be submitted by the Social Worker involved with the family or young person.
For a “good “referral we would like to see some description as to the need of the young person and the impact on the family or young person at this time.

The outcomes to be achieved.
It is about the detail and content really, give us as much as possible so we can begin to build a picture of the need, how to meet the outcomes, matching the best staff etc.
What needs to be in place before a referral is accepted? (for example does a Child’s Plan need to be in place first)
