Who the service is aimed at: 

Children with complex care needs in the community, at home and their families.

Support provided: 

Support children with chronic conditions to manage their treatment and support their health and well being.

Encourage self-management in young people of their own health condition to encourage independence.

Support children with health needs in education; and educate staff to ensure the safety of the children in their care.

Reduce the need for hospital admissions and be a point of contact for any support , questions.

Promote the health of all children and young people by a holistic approach to their needs and signposting to other agencies where needed.

Advocate for children and young people when in our service.

End of Life Care to support children and young people and their families in partnership with the Capella team.

All ages up to 18

All Dumfries and Galloway

Can an individual access the service directly, or do they need to be referred to it by another service – like a school, social worker, health practitioner for example? 

This is a specialist service for children with complex care needs and does not accept self-referrals.  Children would be referred to the service by another service or practitioner.
