How to make a referral: Families should contact the service using the enquiry line 01387 241882 / DG.OT-enquiryline@nhs.scot.  Services should use a Request For Assistance (RfA) form.

Top tips for making a ‘good’ referral (if applicable): Our service works with children and young people between the ages of 0-18 years who have specific difficulties in participating in or completing daily activities.

We do ask that if a professional is referring on behalf of a parent, parental consent is obtained before the request for assistance is sent to us.

What needs to be in place before a referral is accepted (for example a Child’s Plan)?  Nothing
