Can an individual access the service directly, or do they need to be referred to it by another service – like a school, social worker, health practitioner for example?

Another service would make a referral, for example Health Visitor, Allied Health Professional or Nursery.

How to make a referral:

Contact the Council on the telephone number above, and ask for the Educational Visitor Team.

Top tips for making a ‘good’ referral (if applicable):

We have a prompt document for Request for Assistance Forms for both Education and Allied Health Professionals.

This can be requested through Supporting Learners, please highlight this is for the EV Service.

What needs to be in place before a referral is accepted (for example a Child’s Plan)?

We require evidence of Stages 1 and 2 of Stages of Intervention this can be evidenced through assessment/observations/EV Virtual Consultation.

Information about Stages of Intervention can be accessed through GLOW or on request.
